May 18-25, 2010; 6 p.m. to 8 p.m.
at the Dek

Print the proper forms using the

links below and mail it

along with your check or

visit the Dek.

To Register click on the Registration Form & Waiver and Release Form below.
Complete the forms and bring them and your fee to Butler Township Park during registration days or mail to the address below.

Make checks payable to: Butler Dek Hockey
$55.00 per player
Chipmunks: $35.00 per player
